If your DENTIST has arranged for a SAME DAY CONSULTATION AND SURGERY APPOINTMENT, SUBMIT YOUR REGISTRAION FORMS ONLINE by clicking on the link below. You will receive a detailed package of instructions in the mail. This typically takes about 2 weeks to get to you by Canada Post.
The security and privacy of your personal data is our primary concern and we have taken every precaution to protect it.
Please note: Minors must be accompanied by their parent or guardian for both consultation and surgical appointments.
Things to Bring
We are a fee for service practice and accept Visa, MasterCard, Debit cards and cash for payment of accounts.
We currently offer “assignment of benefits” for those with dental insurance. In other words we bill the insurance company first, with the remainder being billed to you after they have paid.
For patients without dental insurance or for services not typically covered by dental insurance (e.g. dental implants) payment is due when the service is provided.
If you have dental insurance benefits, please bring your insurance information so that we can submit to your insurance provider. We will work with you to maximize your insurance benefits and routinely send a pre-authorization for you so that you know what your out of pocket expenses will be.

Alberta Health Care
Some medically necessary procedures may be covered by Alberta Health Care. As a Non-Hospital surgical facility we are often able to perform these in the office rather than in the hospital.
